Your local health board will be able to help you with any FOI requests that are about

  • other NHS health boards
  • medical records
  • dental practices
  • all other healthcare patient-related services.

Please include your full name and address and if possible please also include your telephone number in case we need to contact you about your request. Your request should state as clearly as possible the information you want to receive, including any date ranges.
